I would like your child to take part in our next exam session on MONDAY 22nd DECEMBER at Ravenshead Leisure Centre.

Children will only be entered if attendance is consistent. Lessons and extra exam practices are essential! Failure to do so will result in non-entry. If you cannot commit to extra practice sessions, please let me know, as I need to know numbers before confirming the exam session.

Dancing is a strict discipline and the correct attire is necessary. Children will not be entered if they are not wearing leotard, tights and ballet shoes to classes and exam practices.

Please complete the form below and make payment by bank transfer before Friday 5th December to confirm your entry. Further details regarding times and exam procedure will follow once all applications are in.
